Have you completed Steps 1–4 with a DOT-qualified SAP?

Select “Yes” if you completed your SAP requirements and your SAP has made you eligible to take your Return-to-Duty test.

You need to complete the SAP process first.

Step 5 comes after you have completed the required SAP process. This generally includes:

  • Initial SAP evaluation
  • Completion of the SAP’s education and/or treatment recommendation
  • Follow-up SAP evaluation
  • Being made eligible for Return-to-Duty testing

We will confirm your Return-to-Duty testing requirement with your SAP before coordinating your test.

If your SAP confirms that both a drug test and an alcohol test are required, a separate fee may need to be paid for the additional test. We will let you know before the additional test is arranged.
